Form 000-1 Notice of Candidate Rejection May be sent to an applicant when his application for initiation, affiliation or restoration is rejected.
Form 000-2 Notice of Restoration Sent to Grand Lodge Office upon a brother being restored to full membership after suspension for non-payment of Dues.
Form 101 Declaration of Principles Folds into a handy and instructional pamphlet.
Form 102 Information for Applicants. Freemasonry - Aims and Objectives Folds into an informational pamphlet designed for someone who has expressed interest in the Fraternity.
Form 103 Letter for Potential Applicant This letter must be read and signed by any potential applicant prior to them being issued with a Petition for Initiation..
Form 104-106 Petition for Initiation, Affiliation or Restoration This form must be completed in full, signed by the Petitioner and two brethren of the Lodge handling the Petition.
Form 105 Committee Report on Petition for Initiation The report which is completed and returned to the Lodge Secretary prior to Balloting by the Committee of Investigation on a Petition for Initiation.
Form 107 Committee Report on Petition for Affiliation The report which is completed and returned to the Lodge Secretary prior to Balloting by the Committee of Investigation on a Petition for Initiation.
Form 110 Notice of Suspension Notice sent to member by registered mail when he is suspended for Non-Payment of dues.
Form 111 Certificate of Good Standing This letter certifies that the named member is in "good standing" in the Lodge. It is valid for six months from date of issue.
Form 112 Demit This Certificate is issued when a brother asks for the demit.
Form 120 Notice of Arrears This letter is sent to a brother, by registered mail, when he is "arrears" with his dues. It is sent in the month of October prior to the year end when he will face suspension for non-payment of dues.
Form 124 Statement of Outstanding Dues A simple statement of outstanding dues for a member.
Form 127 Financial Statement Page One ONLY of this form which must be submitted to Grand Lodge by March 15th of the year following the date of the statement. A FULL copy must be sent to the District Deputy Grand Master by the same date.
Form 132 Notice of Affiliation and Dual Membership Sent to the Lodge(s) which provided Certificate(s) of Good Standing for a successful Petitioner for Affiliation into a Lodge.
Form 133 Notice of Affiliation Sent to the Lodge(s) which provided Demit(s) for a successful Petitioner for Affiliation into a Lodge.
Form 160 Statement of the Work This form is to be completed and submitted to the DDGM before March 15th each year. One copy to be retained in Lodge records.
Form 162 Certificate of Election, Appointment and Installation of Officers. This certification of the election or appointment of Lodge Officers and their Installation or Investiture must be submitted to the Grand Lodge Office within 15 days of the Installation.
Form 167 Nomination for Lodge Elective Offices except Tyler For use in a Lodge for the nomination for elected office except Tyler, which is elected "from the floor"
Form 170 Nomination for Grand Lodge Office or Position For use in any elected Grand Lodge Office or Grand Lodge Board or Committee except District Deputy Grand Master. Must be "in the hands" of the Grand Secretary by 12 noon on the last working day of February.
Form 171 Nomination for District Deputy Grand Master For use in the election for District Deputy Grand Master. Must be "in the hands" of the District Nominating Committee Chairman 60 days before the District Meeting.
Form 173 Certificate of Examination of Master-Elect The Certificate completed and signed by the Committee elected to examine the Master-elect as to his proficiency. Must be "in the hands" of the Secretary prior to the commencement of the Installation of Officers.
Form 182.2 Nomination for Masonic Medal of Merit The Masonic Medal of Merit is restricted to a member who has not served as Master of a Lodge.
Form 182.3 Masonic Medal of Merit Nominator's Comment Form The Nominator's comments supporting the nomination which he is making for the awarding of The Masonic Medal of Merit
